Arrival Time: 10-15 minutes prior to appointment to secure time for intake process.

Late Policy:  If arrival is later than 15 minutes of appointment time we will have to reschedule to the soonest next availability.

We will also contact you if cancellations occur in attempt to get an appointment sooner if you request. 

Please bring in a list of medications you take as well as any names and locations of other medical and/or mental health providers you see.
